In this case, Opt Outs may be submitted either electronically via this website or by returning a hard copy in accordance with the instructions set forth on the ballot prior to October 15, 2019.
To submit an Opt Out electronically, please enter (i) your name, (ii) the email address to which you want Prime Clerk to send confirmation of your Opt Out submission and (iii) your Unique E-Ballot ID number in the fields below. Your Unique E-Ballot ID number is printed on the ballot you received in the mail as part of the solicitation package.
You may only use your Unique E-Ballot ID# once. If you would like to change your vote prior to the Voting Deadline or have any questions regarding submitting your ballot, please contact Prime Clerk at 844-232-0772 (domestic toll-free) or 917-942-6394 (international toll) or sheridanballots@PrimeClerk.com for a new E-Ballot ID#.
