Your unique Opt-In ID number is printed on the form you received in the mail.

You may only use your Opt-In ID# once. If you would like to change your Opt-In selection prior to the Global Settlement Opt-In Deadline or have any questions regarding submitting your Opt-In Form, please contact Prime Clerk at (844)-721-3891 (TOLL FREE) or tacobuenoinfo@primeclerk.com for a new Opt-In ID#.
